In order to check out materials from the Library Ministry, you must fill out a library application.
 

All library applications must be signed by an adult (18 years or older).
 

Library applications are not given out to children. An adult must come to the library to sign the card.
 

Each person listed on the library application may be given a library card to be used each time the patron checks out media.
 

To replace a lost library card, 50 cents may be charged.
